Depends entirely on the track. I do whatever I think works for a particular track. I'm not going to avoid a cheesefest if I want it to be a cheesefest, but it can be good to bring some more variation into a track. I never really think "okay, time to put a hard section in now" or anything, it just happens sometimes, and if it doesn't, then fine also.
